Please adhere to the following office policies.
 
Appointments
 
Appointments can be made by calling 646-775-2957
Individual appointments are scheduled for a specific time.  All appointments except for the inital visit are 30 minutes.  Please be on time as your appointment will end promptly 30 minutes after its scheduled start.  Cancellations must be made at least 48 hours in advance by notifying the office.  If you do not cancel your appointment within this time, the cancellation fee is $70, and insurance will not cover this charge.  This is not payment for services in any way, but a cancel charge.  You will still be responsible for the cost of your next office visit.
 

Payment

 

Payment is expected at the time of your appointment.  You may contact our front desk for a fee schedule if your insurance does not cover our services.

Checks are to be made payable to Shannan Carter Nutrition, LLC.  If there is any difficulty in making payment at the time of the visit, please discuss this prior to the initial office visit.

 

Insurance

 

Medical insurance companies may or may not offer coverage for outpatient nutrition counseling, so you must carefully investigate the types of coverage you may have.  At the current time our office participates in some plans with these insurance companies. Although you may have insurance that will reimburse you, if your insurance is not a plan in which our office is participating, you must pay for your visit “out of pocket” and have your insurance company reimburse you if applicable.  In addition, you are required to have a physician referral at the time of your appointment.  It is your responsibility to keep track of the amount of visits you have received.  If the referral expires, or there are no more visits on the referral, you must obtain a new one prior to your next session.  If you fail to keep track of your referrals and are out of visits, you must pay for the session “out of pocket”.  You will be given a form to submit yourself for reimbursement if you choose to do so.

 
If your insurance covers your visit, you must still pay the copay (under "specialist" on your insurance card) and you are individually responsible for the entire amount of the charge for each visit should your insurance company deny coverage for any reason. You may contact our receptionist for a fee schedule in this case.
 
Other
 
Unfortunately because of their committments to other patients, our nutritionists will not be able to assist you over the telephone.  Our office staff should be able to answer most of your questions about the types of services we provide, scheduling and billing.  We do not provide internet, telephone or home care counseling services.
